I can't remember what page it was, but for some reason I thought he died from sepsis? If that's the case, he would have been in tremendous pain up until he died, so I'm thinking I'm misremembering.
Sepsis has been implied until this point since dylan's mum died from it and it's why the hospital shut down.

I guess when you kinda know you are doing bad health wise and seem to be a no-BS person even this is something you just accept. As supernatural as it is. Not that we know what happens after death anyway.

To give jerry maybe a little credit, Malachy might have been wondering about the hospital for a while before he saw Rob and this page is just super condensed, but either way, he didn't need someone to tell him :')

In all honesty I was kinda sold on feeling bad for the dude the second it was revealed that he had to drive himself to the hospital with a ruptured organ. He’s certainly a punkass bitch but there is some depth to that. Complicated characters, I live for it <3

they're free, all necessary healthcare is free I believe (except for like eye/dental checkups). You can pay for private services if you don't want the waiting times or if you want something not offered by the NHS

People have been charged thousands of dollars for shortish rides. It’s a teensy bit fucked
My mom used to drive an ambulance, here in the states. And yeah, she's like "don't take an ambulance unless you 'need' to." The costs are ridiculous for the minimal aid received through them.
Everything about this page hurts. The expressions are on point and that first panel is beautiful. The way the colors shifted from the brights of the last page to this duller, more sickly scheme really piles on the aura of dread. Hella good work.
It really is starting to seem like Jer’s theory is right - ghosts hang around the thing that killed them. Jerry with Dylan, Malachy with this hospital. So...I’m beginning to wonder why, of all places, Rob decided to show up in Corey’s room...
Hmm… I'm starting to wonder if the cause of death was something related to organ harvesting. It would explain why Ms. Harper is an important minor character. She'd effectively be the coroner of the teenaged investigation squad.
Aaaaand it suddenly hits home that dying because some annoying brat decided to mess with your medical equipment when you were sick, then spending a frickin' decade in a living nightmare watching everything around you literally and figuratively fall apart is pretty nightmarish and a reasonable way to drive someone batty enough to… well. Great lengths.
When it comes to torture, everyone has their breaking point. Everyone. The old Hollywood schtick of the protagonist too tough to break is a farce, just ask special ops soldiers who have to endure torture as part of their training. The test is to determine how long you can last before you lose your mind and will do anything to make it end. Under the circumstances that Malachy (getting used to that spelling) has endured, I don't really think it's possible to judge his actions fairly, so I'd basically say that he's a psycho ghost and whatever you can do to get him to move on is an available option. It'd be nice if you could do it in such a way as to give him peace at last, but if you don't know how to do it that would make an already difficult task nearly impossible, or too distasteful (like murdering a bunch of people).
The implication I'm seeing now, as far as the deaths around town are concerned, is that Malachy has possessed people and had them kill those he believes wronged him (Rob and Jerry for starters). If that's the case, then it's no wonder there are a lot of unsolved murders, because the killer probably has no, or only a tenuous, connection to the victim. Then again, Malachy might not be that careful, so who knows?
he was mentioned by Rob in Drain where he says he'll go to his brother in London, which Vic later contradicts saying he has no brother in London
But yeah I kinda considered this the twist!
